A water management passport (WMP) is a document containing information about the water supply and drainage systems of an enterprise with a description of their technical parameters and actual condition at the time of drawing up the passport.

WHAT IS NEEDED FOR A SUCCESSFUL WATER MANAGEMENT AGREEMENT
Normative calculation of water consumption and drainage for the current year, approved by the relevant authority.
Obtain the Act of delimitation of the balance sheet ownership of sewer networks. To do this, contact the Department of Sewerage Operation of Kyivvodokanal (DEKG), located at: Kyiv, Alisher Navoi Ave., 1, tel. (044) 541-94-17. or the Legal Entities Service Center at: Kyiv, Leiptsyzka St., 1-A. tel. (044) 200-73-43.
Agree on the location of the Control Wells on the General Plan (scale 1:500). To do this, contact the DEKG with the original act of delimitation of sewer networks and 2 copies of the General Plan (scale 1:500), to indicate the location of the control wells.
Prepare a package of documents that is included in the water management passport (to download the template, click on the document name).

Rights and obligations of the Company and the Consumer (required to be filled in!)
Act of delimitation of balance sheet ownership and operational responsibility of sewer networks
General plan (M 1:500) indicating outlets and control wells.
5. Approve each form included in the PVG with the signature of the head of your enterprise and a wet seal.
SUBSCRIBER'S FACILITY INSPECTION
To issue a PVG, our specialist must visit your facility for an inspection. During his visit, you must ensure that you have the following documents:
certified copies of title documents for the Subscriber's facility;
certified copies of the Charter and other documents containing information about the Subscriber's main types of economic activity:
certified copy of the EDRPOU certificate, certified by the signature of the head or representative of the Subscriber, or by an authorized person and sealed (if available);
certified copy of the Extract from the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Organizations with information about the Subscriber's managers and types of activities of the Subscriber.
Data on the types of activities of the Subscriber according to the classifier of types of economic activity (KVED) must contain signs of belonging of this activity to the relevant industries (education, health care, culture, physical culture and sports, tourism, art and press, television, radio broadcasting, publishing; sanatoriums and resorts, holiday homes and camps, dispensaries) at the socio-cultural facility.
Information on the absence of tenants or other persons at the Subscriber's facility who discharge wastewater of technological and/or non-domestic origin. This information is checked by the DEN during the inspection of the Subscriber's facility and recorded in the Inspection Report of the facility that discharges wastewater into the city sewer network in accordance with the Acceptance Rules;
A copy of the Act of delimitation of balance ownership and operational water supply and sewage networks on the Subscriber's balance, with the exception of Subscribers who are not connected to centralized water supply and sewage networks;
A copy of the Act of inspection of the Subscriber's facility (if necessary - comprehensive), which contains information on the connection and discharge of the Subscriber's wastewater into the city sewage network or discharge into the network of the main Subscriber;
Technical conditions for the sewerage of the Subscriber's facility and the Act of technical acceptance of the sewage system (if available);
Conclusions of monitoring control over the quality of wastewater discharged by the Subscriber, the regime and volumes of discharge (at the initiative of the DEN);
Other documents regarding water consumption and sewage disposal of the Subscriber's facility, which can reasonably confirm the nature of the Subscriber's wastewater.

CONSIDERATION PERIOD AND VALIDITY PERIOD OF PVG
The term for consideration of documents is 1 month (provided there are no errors and the need to make corrections to the PVG forms).
After approval of the Water Management Passport, the responsible engineer for the district will notify you of the readiness of the PVG for issuance. The Subscriber's representative will need to apply with a power of attorney to the place of submission to receive his copy of the PVG, and submit an application for obtaining the Conditions for discharge.
The passport is valid for the entire period of operation, provided there are no changes in the water drainage of the facility.
Re-issuance of a PVG is necessary in the following cases:
change of the owner of the facility;
transfer from the balance sheet to the balance sheet of buildings and networks;
changes in technological processes;
increase in drainage volumes by more than 30%;
execution of construction works on the territory of the facility.
IMPORTANT! In case of changes in the quantitative and (or) qualitative composition of the wastewater of the enterprise and (or) its sub-subscribers, technological operating modes, as well as any other changes related to the operation of the enterprise's water supply and wastewater systems, Kyivvodokanal must be notified within 7 days.
Please note that from 01.04.2024, the acceptance of documents and their issuance (Agreement for normatively clean water; Agreement for services for the discharge of excessively polluted wastewater into the city sewer; Agreement for conducting wastewater control; Agreement for debt restructuring; Letter for the engineer's departure to inspect the facility; Letter for postponement/granting deadlines; Letter on the implementation of comments/prescriptions of inspection acts; Letter on assigning the facility the status of socio-cultural purpose; Super-normative agreement; Submission of an application for the issuance/extension of conditions for wastewater discharge; Submission of a PVG for registration/re-registration; Information letter (change of details and responsible person, etc.) are carried out exclusively:
- through a personal account for legal entities and the electronic document management service "Signy" (https://my.signy.online);
- at the Consumer Service Center Kyivvodokanal No. 1 (CSO No. 1) at the address: ul. Leiptsyzka, 1-A, 1st floor: Monday-Thursday from 8:00 to 17:00, Friday from 8:00 to 16:00 (lunch break 12:00-12:45). Documents are accepted only through the personal account and the electronic document management service "Signy". The responsible person of CSO No. 1 provides subscribers with assistance in scanning documents into digital form and helps/conducts consultations in the process of uploading them to the electronic account with the mandatory signing of these documents by the subscriber's KEP/EDS.
- at the Consumer Service Center of Kyivvodokanal No. 2 at the address: ul. Elektrotekhnichna, 16, 1st floor, office 117, window No. 1: Monday-Thursday from 8:00 to 16:40, Friday from 8:00 to 15:40 (lunch break 12:00-12:45). Documents are accepted only through the personal account and the electronic document management service "Signy". If the subscriber wants to submit documents through the account and the electronic document management service "Signy", the responsible person from the environmental supervision department provides subscribers with assistance in scanning documents into digital form and assists/provides consultation in the process of uploading them to the electronic account with the mandatory signing of these documents by the subscriber's CEP/EDS.
Note* documents sent via email or left in a mailbox on the first floor or signed with a qualified electronic signature (QES)/electronic digital signature (EDS) by another person or another enterprise will not be registered.
