PJSC "AK "Kyivvodokanal" on the conclusion of
PUBLIC INDIVIDUAL CONNECTION AGREEMENTS
on the provision of centralized water supply and/or
centralized water disposal services
In connection with the adoption of the Resolution of the Cabinet of Ministers of Ukraine dated 29.12.2023 No. 1405 “On Amendments to Certain Resolutions of the Cabinet of Ministers of Ukraine on Payment for Housing and Communal Services”, which amended the Rules for the Provision of Services for Centralized Water Supply and Centralized Water Drainage and Standard Agreements for the Provision of Services for Centralized Water Supply and Centralized Water Drainage, approved by the Resolution of the Cabinet of Ministers of Ukraine dated 05.07.2019 No. 690, and guided by the norms of Part 5, Article 13 of the Law of Ukraine “On Housing and Communal Services”, Art. Art. 633, 634 of the Civil Code of Ukraine, PrJSC "AK "Kyivvodokanal" publishes this public proposal (offer) for the conclusion of:
- a public individual contract for the provision of services for centralized water supply and/or centralized water drainage for consumers-owners (co-owners, users) of residential and non-residential premises in an apartment building, other real estate objects or water supply and/or water drainage in an apartment building connected to intra-building networks, who receive services to meet their own needs, as well as to ensure the economic, technological (own) needs of the apartment building as a whole;
- a public individual contract for the provision of centralized water supply and/or centralized wastewater services for consumers-owners (users) of individual (manor) houses that are connected to centralized water supply and/or centralized wastewater networks in accordance with the requirements of the Rules for the Use of Centralized Drinking Water Supply and Centralized Wastewater Systems in Settlements of Ukraine, approved by the Order of the Ministry of Housing and Communal Services of Ukraine dated 06/27/2008 No. 190 (as amended by the Order of the Ministry of Community and Territorial Development of Ukraine dated 04/19/2021 No. 97).
The agreement concluded with the co-owners of apartment buildings is considered concluded if, within 30 days from the date of publication of the text of the agreement on the website of PrJSC "AK "Kyivvodokanal", the co-owners of the apartment building have not made a decision on the choice of a model of contractual relations and have not concluded a corresponding agreement with the Company.
The agreement concluded with the owners (users) of individual (manor) residential buildings is considered concluded if, within 30 days from the date of publication of the text of the agreement on the website of PrJSC "AK "Kyivvodokanal", such an owner has not taken any action to disconnect (refuse) from centralized water supply and/or centralized wastewater services.
Due to the fact that these agreements are connection agreements, their terms are not subject to agreement with the connecting party (consumer).
The contracts are posted on the website of PrJSC "AK "Kyivvodokanal" in free access and in a manner that ensures familiarization with their contents by every person who receives and applies to the Company to receive centralized water supply and/or centralized wastewater services.

In compliance with the requirements of the Resolution of the Cabinet of Ministers of Ukraine dated 02.02.2022 No. 85 “On Amendments to the Resolution of the Cabinet of Ministers of Ukraine dated July 5, 2019 No. 690”, PrJSC “AK “Kyivvodokanal” publishes this public proposal (offer) for the conclusion of:
- a public individual contract for the provision of centralized water supply and/or centralized water drainage services for consumers - owners (co-owners, users) of residential and non-residential premises in an apartment building, other real estate objects or water supply and/or water drainage in an apartment building connected to intra-building networks, who receive services to meet their own needs, as well as to ensure the economic, technological (own) needs of the apartment building as a whole;
- a public individual contract for the provision of centralized water supply and/or centralized wastewater services for consumers - owners (users) of individual (manor) houses connected to centralized water supply and/or centralized wastewater networks in accordance with the requirements of the Rules for the Use of Centralized Drinking Water Supply and Centralized Wastewater Systems in Settlements of Ukraine, approved by the Order of the Ministry of Housing and Communal Services of Ukraine dated 06/27/2008 No. 190 (as amended by the Order of the Ministry of Development of Communities and Territories of Ukraine dated 04/19/2021 No. 97).

A contract concluded with owners (users) of individual (manor) residential buildings is considered concluded if, within 30 days from the date of publication of the text of the contract on the website of PrJSC "AK "Kyivvodokanal", such owner has not taken any action to disconnect (refuse) from centralized water supply and/or centralized wastewater services.
Due to the fact that these contracts are connection contracts, their terms are not subject to coordination with the connecting party (consumer).
The contracts are posted on the website of PrJSC "AK "Kyivvodokanal" in free access and in a manner that ensures familiarization with their contents by each person receiving and applying to the Company to receive centralized water supply and/or centralized wastewater services.

HOW TO CONCLUSION A CONTRACT?
A consumer who intends to conclude a contract for water supply and wastewater services with PrJSC "AK "Kyivvodokanal" draws up and submits the initial package of documents:
List of documents required for concluding contracts for the provision of centralized water supply and/or wastewater services in the private sector:
Application for conclusion of the contract (download the form to fill out, download the list of documents required for conclusion of the contract);
for an existing facility - a copy of the document certifying the right of ownership or use of the water consumption facility (extract from the register of rights, purchase and sale agreement, certificate of ownership, act of acceptance - transfer, lease agreement, etc.);
for construction - a copy of the permit or declaration for construction work, a document for a land plot (state act for land, purchase and sale agreement, donation agreement, etc.);
for newly built or reconstructed facilities - a copy of the document on the completion of construction work (act of readiness of the facility for operation, declaration on the commissioning of the building, etc.);
Copy of the consumer's passport 1, 2 and the page indicating registration;
Copy of the certificate of assignment of an identification number;
Notarized power of attorney for an individual who has the right to receive, provide, and execute relevant documents for concluding an agreement, as well as the right to sign and conclude an agreement, a copy of the authorized person’s passport (if the agreement is signed by the authorized person of the consumer);
A copy of the technical passport for the house;
For subconsumers – in case of connection to the networks of the main consumer, who is their owner, a written consent is provided;
A copy of the manufacturer’s passport for the metering device, if available – a certificate of verification. A copy of the type verification certificate and a copy of the certificate of conformity or a copy of the type verification certificate and a copy of the certificate of approval of the quality management system (for each device). If the meter is missing, not verified, not working or the documents are lost – you can contact the Kyivvodokanal Service Center;
If available - Copy of the technical acceptance certificate of water supply networks and the technical acceptance certificate of sewage networks (for newly built facilities - mandatory), or a general plan (fragment) 1:500, or a situational plan;
Copies of documents for obtaining benefits (list according to current legislation).
